Our Services
We are committed to providing individualized support services that empower individuals with developmental and intellectual disabilities to live fulfilling, independent lives. Through our Supported Living Services (SLS) and Self-Determination Program (SDP), we work closely with clients and families to create personalized support systems built around each individual’s unique goals, needs, and level of independence.
Empowering Individuals to Live Independently
Supported Living Services (SLS) are designed to help individuals with developmental and intellectual disabilities live safely and independently in homes of their choice while remaining active members of their communities.
SLS may include assistance with household management, budgeting, meal preperation, medication support, transportation, communication skills, community integration, and personal growth. The goal is to provide the right level of support while encouraging independence and long-term success. These services are personalized to each individual’s needs and goals, promoting dignity, choice, and meaningful daily living.
We have successfully onboarded multiple SLS cases while maintaining a strong commitment to quality care, consistency, and individualized support. Through careful planning and coordination, we have been able to provide adequate staffing tailored to each individual’s unique needs, ensuring continuity of care and meaningful community integration.
Choice, Freedom, and Personalized Support
The Self-Determination Program (SDP) is a voluntary program that gives individuals with developmental and intellectual disabilities greater control over the services and support they receive. Through SDP, participants are empowered to make decisions about their goals, daily lives, and the providers they choose to support them.
Instead of relying solely on traditional services models, individuals participating in SDP receive an individualized budget that can be used to purchase services, supports, and resources tailored to their unique needs and preferences. This flexible approach encourages independence, creativity, and person-centered planning.
We have successfully assisted multiple individuals and families with smooth transitions into SDP. Through personalized guidance, collaborative planning, and ongoing support; we help ensure that each transition is seamless, person-centered, and tailored to the unique goals and needs of the individual. Our team is committed to empowering participants with the tools, resources, and support necessary to thrive within SDP.
We are a proud member of the Ramona Chamber of Commerce, reflecting our commitment to supporting local businesses, community collaboration, and the individuals and their families we serve.
Although our home base is in Ramona, we are committed to serving individuals and families throughout all of San Diego and Imperial County with compassionate, reliable, and community based support services.
We have developed dedicated teams located across Ramona, La Mesa, Santee, Poway, and El Cajon; and we are eager to build in the next location!
